The Richmond Response and Rehabilitation Team provides a lively multi-disciplinary service for adults with acute physical conditions, living in the affluent, residential district of Richmond borough, bordering the River Thames.The clinical coordinator will be responsible for clinical triage of referrals into the RRRT service. This role may include and not be exhaustive of: clinical decision making, signposting referrals/communicating with external agencies, making direct referrals or offering clinical advice.The post holder will work with the senior clinical co-ordinator, screening referrals to ensuring they follow the correct pathway (D2A, Urgent Care Reponse (UCR) or Community Rehabilitation) with a person centred approach.Main duties of the jobThe clinical coordinator will support the senior clinical coordinator to ensure that all referrals to RRRT are triaged and managed appropriately within agreed time scales.You will need to be a professionally qualified AHP, paramedic or registered nurse with significant experience in the community sector.You will need excellent communication and organisational skills and be scrupulous about documentation.Above all, you will need to passionate about improving outcomes for patients.About usCommunity healthcare is unlike any other part of the NHS.
