Real Staffing are seeking an experienced interim Children's Continuing Healthcare Nurse Assessor/ Case Manager to support a NHS Integrated Care Board on a remote basis.

Key Responsibilities:

  • Assessing healthcare needs for children requiring ongoing care
  • Collaborating with healthcare professionals, families, and caregivers to develop comprehensive care plans
  • Monitoring and evaluating the effectiveness of care plans, advocating for necessary adjustments when needed
  • Providing support and guidance to families navigating the healthcare system
  • Ensuring compliance with regulatory standards and guidelines
  • Maintain accurate records and documentation

Requirements:

  • Experience in complex CYP case management
  • Qualified Children's Nurse with CHC experience
  • Excellent communication and interpersonal skills
