We have an exciting opportunity for a Band 5 Children and Young People's Continence Nurse to join our well-established, national award-winning Children and Young People's Continence team, based in Lancing. The post is for 22.5 hours a week across 3 working days, within our integrated team of Continence Nurse Specialists, Continence Nurses, Nurse Associates and Associate Practitioners. The post holder will have a key role in delivering children's continence care for the children and young people on the enuresis caseload and will need to have the ability to travel to clinic venuescountywide across West Sussex and Brighton and Hove. Manage a clinical caseload of children with continence care needs, providing continence clinics face to face and virtual, across a variety of settings, to meet the needs of the service. Undertake assessments and reviews to identify the child/young person's specific continence needs, developing and implementing detailed care plans reflecting interventions and case management as per NICE guidance including medication recommendations Support and educate staff offering relevant clinical knowledge and skills for the delivery of continence care for children within the CYPCT caseload, facilitating access to expert specialist knowledge and skills as needed Ability to communicate with our wider partnership stakeholders and other professionals involved in the child or young persons care working closely with social care and education. Protect, safeguard and promote the well-being of children and young people in accordance with Trust policy and national guidance Keep accurate, legible, contemporaneous electronic records of all care interventions, written reports and referrals as required Demonstrate proficient knowledge of information governance Help develop, implement and review clinical protocols and standard operating procedures which assist in delivering safe care and best nursing practice

Job descriptionJob responsibilitiesAs a Registered Nurse the Children's Continence nurse will work as part of the Children& Young Peoples Continence Team supporting children and young people with enuresis (night wetting), promoting health and wellbeing and ensuring the provision of high quality, safe nursing care to children and families on the caseload.
Manage a clinical caseload of children with enuresis (night wetting) providing nurse-led continence clinics in a variety of settings, offering face to face and virtual consultations Undertake initial assessments and review appointments to identify the child/young persons specific continence needs, developing and implementing detailed care plans reflecting interventions and case management as per NICE guidance including medication recommendations Promote and demonstrate best practice, integrating evidence based practice and clinical reasoning into care delivery. Assist with the implementation of national standards and guidance on childrens continence care Organise, plan and prioritise work according to service needs with a responsibility for supporting other team members regarding service delivery. Ensure health records are maintained and compliant with NMC and Trust policy and keep accurate, legible, contemporaneous records of all care interventions, written reports as required, with proficient knowledge of information governance.
Provide caseload monitoring and support to team lead, to aid analysis of relevant service data to demonstrate activity of the team and assist with identifying service needs and possible developments. Be responsible for maintaining clinical standards and participate in audits to ensure a high standard of practice is achieved, at both service and Trust wide level
Help develop, implement and review clinical protocols and standard operating procedures which assist in delivering safe care and best nursing practice. Work within Trust safeguarding procedures, reporting and acting on any concerns appropriately and undertaking statutory and additional training required for the role. Recognises own professional boundaries and develops awareness of roles and areas of expertise of other health professionals and agencies Responsibility for own professional development attending mandatory and other training as agreed in the PDR process and as required by the Trust Support and educate staff offering relevant clinical knowledge and skills for the delivery of continence care for children within the enuresis caseload, facilitating access to expert specialist knowledge and skills as needed Develop a working knowledge of the Trust Children & Young Peoples Continence Policy and provision of products Report clinical incidents via the correct channels, ensuring learning and implementation of relevant recommendations. To demonstrate an awareness of budgetary restraints with responsible use of resources regarding Trust materials and equipment those pertinent to the team include portable bladder scanners, enuresis alarms and accessories Staff may be required to work anywhere in West Sussex or Brighton & Hove in order to meet the needs of the service
