Care Navigator

We are looking for customer focused individuals, ideally with some primary care experience to work as a Care Navigator for our client in Bracknell. As a Care Navigator you will be answering telephone calls from patients to reach the most appropriate outcome for their care need.
This is a busy role, in a team of 16. There are excellent opportunities to progress with the company. The role is based on 4 sites around Bracknell, so ideally you will be a driver.
Key Responsibilities
Answering all incoming patient calls
Using integrated systems to-
Book appointments with the appropriate clinician
Signpost patients to alternative services or information sources
Dealing with patient queries, primarily over the phone
Administration duties and data entry
Rota'd time of front desk at each site dealing with walk in patient appointment requests/other queries and/or requests
Key Requirements
Previous primary care experience is desirable however experience within a customer service or call centre environment is also suitable
Good communication skills
Good customer service skills with a polite and personable manner
Strong organisational skills with an ability to follow procedures and multitask
