Job Title: Care Coordinator

Location: Barnsley

Salary: �24,336 per annum

Employment Type: 37.5 hours per week currently the shifts vary between 8am and 8pm



A reputable healthcare service provider is seeking a dedicated and experienced Care Coordinator for their Barnsley team. We are currently seeking a skilled and experienced Care Coordinator to join our team and play a crucial role in ensuring the well-being of our clients.



As a Care Coordinator, you will be responsible for organizing and managing the delivery of care services to our clients, ensuring they receive the highest standard of care. You will play a pivotal role in matching our clients with appropriate caregivers, managing schedules, and maintaining effective communication between clients, caregivers, and the management team.



Responsibilities:




  • Organise and manage care services for clients, ensuring high standards.

  • Match clients with suitable caregivers, fostering positive relationships.

  • Maintain accurate records of care plans, schedules, and service delivery.

  • Monitor and review caregiver performance, ensuring regulatory compliance.

  • Respond promptly to client and caregiver inquiries and concerns.

  • Collaborate with the management team to optimize operations.

  • Conduct client assessments and reassessments, adjusting care plans as needed.

  • Ensure confidentiality and privacy of client and caregiver information.



Qualifications and Skills:




  • Preferably, prior experience in care coordination or a similar role.

  • Background in healthcare, social work, or related fields is advantageous.

  • Strong organizational and time-management skills.

  • Excellent communication and interpersonal abilities.

  • Proficient in scheduling and record-keeping software.
