Camhs Urgent Care Band 5 ASD nurse. An opportunity has arisen for a Band 5 RMN/RNLD in Brent and Harrow Camhs Urgent Care team. The candidate will work alongside the Camhs Urgent Care ASD Lead in providing support to children and young people attending A& E in crisis. The successful candidate will receive regular clinical supervision, and relevant training to enhance their skills in managing children with neurodiversity.Main duties of the jobWork in a multidisciplinary team. Liasion with other services. Work in collaboration with community teams, social care, A& E's and schools. Assessment of children and young people attending A& E in crisis. To undertake initial holistic and risk assessments of young people presenting in crisis with challenging behavior in the context of their autism. These will take place mainly in A& E and on the pediatric wards, crisis hubs, HBPoS suites and adult wards and facilitate hospital admission and/or discharge into the community with a robust package of care. To consult with staff at the Community CAMHS teams to provide further specialist assessment if necessary. To follow the assessment protocol for such cases and to complete the clinical record on SystemOne (electronic patient records).
Short term crisis intervention:
To formulate and implement care packages for the effective support of the clients neurodivergent needs. These should be based on an appropriate conceptual framework and evidence-based models of practice. To evaluate and make decisions about treatment options in consultation with the multi-disciplinary team, taking into account both theoretical and therapeutic models. To act as a co-worker with other members of the multidisciplinary team as appropriate. The aim of the service is to avoid admission into mental health wards. The post holder will need to attend network meetings and advocate for the young person to ensure this. However, if an admission into mental health wards occur to continue to provide care for all young people admitted to In-patient mental health wards taking on the role of Care Coordinator and participating in CRAFT meetings within seven days following admission and subsequent CPAs, supporting earliest possible discharge from hospital to intensive community care and treatment alongside CAMHS.
Transfer of client to local CAMHS on completion of assessment and/or treatment:
To provide advice, guidance and consultation to professionals working in other agencies and contribute to multi-agency care planning. To ensure effective verbal and written communication with all staff and clients in order to maximise user involvement in the delivery of care and treatment on transfer to appropriate UCOS/ local CAMHS, Local Authority and/or Education.
At all times:
To maintain clinical records appropriately and in line with the Trust policy. To comply with London Child Protection Procedures on the management of child protection issues as well as Central and North West London NHS Foundation Trust internal procedures.
