Successful candidates must have relevant clinical background, ability to integrate well into a multidisciplinary working environment and be a car driver/If you would like to know more, please contact: ##### duties of the jobWorking in the hospital-based multidisciplinary Neck of Femur Early Supported Discharge (NOF ESD) Team:To provide specialist treatment and advice to patients within the community setting, facilitation and maximising patient potential within the service boundariesTo give highly specialised guidance to other members of the MDT and GPs on matters relating to your specialist area eg. the nursing, and rehabilitation needs of patients discharged following surgery into their own home environment.Provide leadership for the NOF ESD team, in conjunction with the Team Lead, in planning, co-ordinating, delivering and evaluating service provided to patients within your specialist area on a day-to-day basisTo provide a role in teaching and supporting colleagues in matters relating to your specialist areaTo participate in the training of undergraduate nurses and non-qualified staffTo deputise in the absence of the Team Lead as requiredTo contribute to the day to day running and development of the service through promotion, adaptation and development of policies, procedures and guidelines using evidence-based practice and current researchAbout usWe provide care for the residents of three diverse London boroughs.
