The post holder will demonstrate a sound knowledge of the disease area and have the ability to coordinate clinical issues across the Trust and represent those views both internally and externally as required.• To work in an advanced practice role clinically within Rheumatology andliaise closely with the research nurses/clinical trials team.• To work with the wider rheumatology team to deliver a specialistrheumatology nursing service.• To manage a defined caseload of patients through nurse led clinics and apatient help-line.o assessing patient needs and prioritieso providing patient education, advice and support to self-manage theirconditiono enabling patients to make informed decisions about their careo planning, implementing and evaluating care and outcomeso working with existing protocolso maintaining patient safety To work with the rheumatology team to regularly update and develop
