Fixed Term Band 5 Physiotherapist to join our Team. The post offers a 6 monthly rotational programme working across a variety of different areas including Urgent Care, Stroke, Surgery, Orthopaedics, Neuro Rehabilitation, MSK Outpatients and Intermediate Care Services in the community
The physiotherapist will act as an autonomous practitioner both professionally and legally accountable for all aspects of their professional and clinical work Assess and treat patients promoting timely hospital discharge with a home first ethos. You will workas part of an integrated multi-disciplinary therapy team to improve flow and quality of a patient's journey. The caseload will be variable and comprise of a variety of conditions. Optimise the clinical progress and quality of life of patients by providing individualised physiotherapeutic assessments, evidence-based recommendations and treatment plans across designated wards and clinics. Work as a committed member of the physiotherapy team, contributing to the achievement of Departmental, Team and Trust goals. Provide relevant advice and training to health care professionals on speciality. Participate in service development. Provide specialist knowledge across a range of procedures underpinned by National standards and theory. To participate in the Respiratory On Call evening and weekend duties this is underpinned by an educational and competency assessed programme.
Main duties of the job
Provide a clinical physiotherapeutic service to designated wards, outpatient clinics, intermediate and community services at Stockport NHS Foundation Trust. Attend relevant ward rounds/meetings such as whiteboards, and liaise with medical, paramedical, nursing, discharge and social work professionals as necessary. Act as a resource for physiotherapeutic information and advice to other professionals. Assist with training and updating healthcare professionals, regarding evidence-based therapy, including giving lectures and informal talks. Collect all data necessary to meet local information requirements and as required by the Therapy service for monitoring. Participate in quality assurance programmes, and development and maintenance of service standards, policies & procedures. Participate in personal and professional development through objective setting and regular job review. Work as a committed and effective member of the physiotherapy staff in order to maximise the potential of the service and profession. Maintain professional competence and expertise by keeping up to date with current research, working practices and models of working. Provide physiotherapy cover for other physiotherapy teams as necessary. Partake in the respiratory on call overnight and weekend service.

To undertake Physiotherapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the patient in all areas of activities of daily living. To work with patients to identify physiotherapy goals as part of the overall patients treatment. To plan and implement individual interventions, in collaboration with the patient, using graded activity to achieve therapeutic goals. Analyse and evaluate the outcome of treatment programmes with flexible adjustment of goals where necessary, in order to ensure effective intervention To carry out home assessment / environmental visits as appropriate. To assess safe and timely discharge of patients from hospital using the discharge to assess model as appropriate. Making onward referrals when appropriate Ensuring smooth transition upon discharge to home/alternative placement by providing recommendations regarding physiotherapy to carers, relatives or nursing home staff To plan and prioritise workload under supervision of a senior member of staff. To be professionally and legally accountable for all aspects of your own work. To comply with the HCPC guidelines. To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. To move and handle patients and equipment safely in accordance with Trust and national guidelines. To demonstrate on-going personal development through participation in internal and external development opportunities, recording learning outcomes in a portfolio. To appropriately manage any physical contact with bodily fluids, e.g, vomit, urine, faeces, in line with Trust policies, COSHH and Infection Control, if this occurs during an occupational therapy intervention. Offer specialist advice to colleagues from other disciplines Seeking second opinions when necessary

If successful this vacancy will require an enhanced DBS check at a cost of 41.90. Degree in Physiotherapy. State Registration with the Health Professions Council.

Knowledge of current best practice in physiotherapy Basic knowledge of health legislation and current practice, risk assessment and management. Basic knowledge of the principles of clinical governance including audit Specialist knowledge across a range of physiotherapeutic procedures underpinned by theory
