This programme has created some fantastic new opportunities for Advanced Practitioner Social Workers across different teams within Adult Social Care.

About the role
As an Advanced Practitioner Social Worker, you will respond to individuals in our community to provide social care support and utilise appropriate interventions. You will use a strength-based approach to promote the independence, interests, and rights of our residents and their families.
As an Advanced Practitioner, you will also be able responsible for managing more complex cases and supervising a small number of staff.
Across our Adult Social Care service, we strive to deliver a culture of working together, positive risk-taking, and accountability. Our core aim is to support our residents to be influential, active members of our community.
As part of your Continued Professional Development you can choose a pathway to support your career interests and ambitions. Opportunities are also available to train as an Approved Mental Health Professional, Best Interest Assessor and Practice Educator.
About you
You will be a qualified Social Worker and registered with Social Work England. You will also have experience of working within social care or a community-focused multi-disciplinary setting.
A good understanding of relevant Adult Social Care legislation is essential for success in these roles. You will have the opportunity to put this legislation into action, further enhancing your skills to support and empower our service users to live meaningful lives within the community.
You will be able to work independently to deadlines and adapt to changing circumstances, as well as demonstrate skills in:
Communication
Prioritisation
Caseload management
Time management
Multi-disciplinary working
We will provide training for an exceptional candidate who cannot demonstrate all of the above criteria.

Adult Social Care is part of the Care Health and Wellbeing department, and together we have a clear focus on working together to support residents, families, and communities.
The department brings together a range of services and support. This enables us to have a clear focus on accommodation as a building block for long-term wellbeing and independence, as well as deliver genuinely multi-disciplinary working for people of any age with more complex needs.

Additional information
Salary Details - PO3 roles attract a golden hello payment of £5000 and a annual retention payment of £3,000 per annum. Additional payments are available for Practice Educators, Best Interest Assessors and AMPs.
We have positions available in the hospital discharge team.
We will be regularly reviewing applications and conducting weekly interview panels.
