Job DescriptionAdvanced Clinical Practitioner, SeftonAre you an enthusiastic and dedicated Advanced Clinical Practitioner and a registered Nurse or AHP or Pharmacist?gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England. An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Sefton Acute Visiting Service.The Sefton Acute Visiting Service responds to both own home and care homes. Our service has two elements, a same day GP booked acute visiting service and a two hour community response.The successful candidate will hold an MSc in Advanced Clinical Practice or Credentialing qualification and a Non-Medical Prescribing qualification. You will be able to work within the four pillars of practice. As an Advanced Clinical Practitioner you will act as a positive role model supporting junior staff whist working flexibly between urgent care and primary care, where appropriate and within a job plan, to provide safe, clinical and expert care.Our population is diverse and multi-cultural with a strong community spirit. A high percentage of our patients face deprivation and multi morbidity, this makes for a varied role as well as an opportunity to make a real different to individuals, families and the wider community.We welcome applications from both newly qualified and experienced Advanced Clinical Practitioners, support and development via our gtd training academy will be provided.As an Advanced Clinical Practitioner, your responsibilities and duties will include:Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required).You will be supported by the non clinical team who have a good knowledge of our extensive local hospital and community services.The clinical team includes GP's, Practice Nurse, Pharmacist, Paramedic and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.Assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff.
