Adults Social Worker - Hospital Discharge - Queen Alexander Hospital

An exciting opportunity has arisen for a Qualified Social Worker to join our friendly and supportive hospital discharge team at Queen Alexander Hospital discharge team in Portsmouth. Position curretly available until June 24.

Responsibilities:

• Working with the acute team.
• The focus will be on supporting the turn around of patients at the front door and working with the Single Point of access here at the hospital.
• This role requires you to be on site 4 days a week as you will be assessing patients for discharge planning.
• 1 day a week will be working remote covering another hospital.
