Full Time / Permanent An exciting opportunity has arisen and we are looking for a highly experienced, motivated, dynamic and energetic Team Manager who can lead London Borough of Merton’s Transitions Team, with a passion for making a difference. The Transitions Team works with young people (14 – 25 years) with complex lifelong disabilities who will go on to require support from Adult Social Care. Currently, eligible young people transfer to the team at the age of 16 years and the team deliver a range of interventions. The work is complex, varied and interesting, allowing for a high level of multi-agency working, creativity and autonomy. The Team Manager is responsible for the operational leadership and management of the Transitions Team working with young people with Learning Disabilities and/or Autism and their carers. This includes the commissioning, assessment, planning and the provision of health and social care services. As a competent, Team Manager, the successful applicant will be responsible for the overall performance and service delivery of the Transitions Team. Prior supervisory experience is therefore essential, in addition to having a strong safeguarding background, a detailed understanding of SEND, EHCP, and a good working knowledge of both children’s and adult’s legislation. An advanced knowledge and understanding of current (the Children Act, Children and Families Act, Mental Capacity Act and Care Act 2014), safeguarding adults and impending legislation and statutory guidance relating to social care and health provision. The Team Manager will also be responsible for chairing multi-agency meetings as required and represent the department at inter-disciplinary and strategic meetings. Responsible for the staffing budget and operational expenditure pertaining to the team’s activity, and for the performance of the team overall. Candidates should have extensive management experience including, inter-agency and partnership development and the development of practice standards. You will have excellent communication and negotiation skills to work with carers and providers to maintain focus on outcomes for service users, in line with the council’s statutory duties. Applicants must hold a professional qualification in either Social or Health Care to degree level with current registration with relevant professional body.
