Northumberland County Council's Adults Community Team is seeking a dedicated and experienced Social Worker to join our team. We are committed to providing high-quality, person-centered support to adults with care and support needs, enabling them to live fulfilling and independent lives.

About the Role:

As a Social Worker in our Adults Community Team, you will play a vital role in supporting individuals to maintain their independence and well-being. You will:


  • Conduct comprehensive assessments of individual's needs: Identifying their strengths, challenges, and aspirations.
  • Develop and implement individualised care plans: Ensuring tailored support is provided based on their specific needs and preferences.
  • Work closely with individuals, their families, and other professionals: Including health care providers, housing providers, and community organisations, to create a holistic support network.
  • Advocate for individual's rights and needs: Ensuring they have access to the appropriate services and resources.
  • Promote independence and well-being: Supporting individuals to make informed choices and maintain their quality of life.
  • Maintain accurate and up-to-date records: Adhering to relevant legislation and policies.

Who we're looking for:

We are seeking a highly motivated and empathetic Social Worker with a strong commitment to empowering adults to live fulfilling lives.

You will have:


  • A recognised social work qualification (e.g., DipSW, CQSW, BSC Social Work)
  • A valid Social Work England registration
  • Experience in working with adults with care and support needs, including experience in conducting assessments and developing care plans.
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse individuals.
  • A deep understanding of relevant legislation, policies, and procedures relating to adult social care.
  • A commitment to promoting the well-being and independence of adults.
