Place of work: Devon-Newton Abbot- Office based
We offer: £50-55K (Full time) per annum -depending on experience, 23 days annual leave + bank holidays, pension, salary sacrifice schemes, benefits programme, free parking, and more!

Responsibility
What will you be responsible for:
Coordinating comprehensive care for children with complex health conditions, ensuring they receive the right services.
Specifically the activities will be:
Assessment, care plan development, coordination with providers, patient advocacy, and excellent communication.
Experience

Experience in complex care management is a plus!

NMC PIN, Children's Nursing qualification.
