Healthy communities are underpinned by better mental and physical health. However, not all aspects of community life are available to all adults - social isolation, unemployment, financial and housing problems, socioeconomic deprivation and domestic issues can all have a negative impact on a person's mental and physical health. Mental Health Community Connectors in Rotherham play a key role in engaging and supporting people to overcome the barriers that they may experience, enabling them to access community support and services to improve their health outcomes.
Mental Health Community Connectors are part of the transformation of community mental health care in Rotherham, helping the NHS to move towards integrated community mental health services across primary and secondary care in collaboration with Rotherham Local Authority and the Voluntary and Community Sector (VCS). This means services are brought closer together, more responsive, easier to access and achieve longer lasting and better health outcomes for patients.
