Lead Practitioner - Homelessness Support

As the Lead Practitioner within their team, you'll assume a multifaceted role incorporating both Visiting Housing Support and Preventative Support.
Key Duties and Responsibilities:
- Visiting Housing Support:
- Assist individuals in navigating housing applications.
- Provide guidance and support throughout the housing procurement process, ensuring individuals have access to suitable accommodation options.
- Collaborate with individuals and their support networks to address housing-related challenges and develop sustainable housing plans.
- Offer practical assistance in settling into new accommodation, including coordinating moving logistics and addressing any immediate housing needs.
- Preventative Support:
- Employ a person-centered approach to engage with individuals and their support networks, including family, friends, healthcare professionals, advocacy services, and team members.
- Provide proactive support aimed at preventing homelessness, such as budgeting assistance, financial management guidance, and access to benefits.
- Foster independence by empowering individuals to manage day-to-day household responsibilities effectively.
- Actively engage in ongoing training and professional development opportunities to enhance skills and knowledge relevant to the role.
