Job descriptionPurpose of the Role:As an Clinical Lead, you will play a crucial role in providing specialist one-to-one support, guidance, and practical solutions for families, focusing on three Dorset-based care homes . Dementia UK will support you in maintaining and strengthening your clinical and leadership expertise through regular clinical supervision and professional development opportunities.Key Duties and Responsibilities: Dementia Expertise: Stay updated on the latest research, treatments, and best practices in dementia care to provide specialised knowledge and expertise. Assessment and Care Planning: Conduct comprehensive assessments, develop personalised care plans, and consider the physical, emotional, and social well-being of individuals. Emotional Support: Offer emotional support to individuals and their families, providing a listening ear, guidance, and reassurance during difficult times. Education and Advice: Provide education and practical advice to staff and families, helping them understand dementia and its progression. Coordination of Care: Collaborate with healthcare professionals to ensure a holistic approach to care, coordinating services and facilitating access to support networks.
