Children with Disabilities Team

We are seeking a passionate and dedicated Children's Social Worker to join our established and supportive Children with Disabilities Team in Stockton-on-Tees. This is a rewarding opportunity to make a real difference to the lives of children and young people with disabilities and their families.
Responsibilities:

  • Direct work with children and families: Provide direct support and guidance to children with disabilities and their families, addressing their individual needs and challenges. This may include practical support, emotional support, advocacy, and signposting to other services.
  • Assessment and Care Planning: Conduct thorough assessments of children's needs and develop person-centred care plans, working collaboratively with families and other professionals.
  • Multi-Agency Working: Work effectively with a range of professionals, including health professionals, educational professionals, therapists, and other support services, to ensure holistic and coordinated support for children and families.
  • Advocacy: Advocate for children's rights and needs, ensuring they have access to appropriate services and resources.
  • Case Management: Manage a caseload of children and families, ensuring timely and effective service delivery.
  • Record Keeping: Maintain accurate and comprehensive records of all contacts and interventions.
  • Training and Development: Participate in relevant training and development opportunities to enhance professional skills and knowledge.

Essential Criteria:

  • Qualifications: Relevant professional qualification (e.g., social work, youth work, or equivalent).
  • Experience: Experience working with children with disabilities and their families.
  • Skills: Excellent communication, interpersonal, and problem-solving skills. Ability to build rapport and trust with children and families. Ability to work independently and as part of a team.
  • Knowledge: Understanding of the needs of children and families affected by disability, and relevant legislation.
