£16.03-£17.89 per hour
Mileage paid at 45p per mile
Increased pay rates for evenings, weekends and bank holidays

About the role:
We are looking for a dynamic and enthusiastic person to join our Reablement Team, providing support for our colleagues across Dorset. We support people who have recently bee discharges from hospital, but still need support with daily activities.
You will be supporting our Reablement Assistants in their day-to-day role, performing risk assessments and supervisions. You will also conduct regular admission assessments and care plan reviews.
Responsibilities and accountabilities:
Supervise a team of Reablement Assistants providing reablement support to service users in their own homes.
Ensure that staff managed are meeting Essential Standards of Quality and Safety and are working in accordance with directorate policy.
Provide readily available information or assistance, dealing with issues where the outcome may not be straightforward.
Monitor working practices and quality assurance systems providing support and guidance to Reablement Assistants where necessary, within Care Dorset structures and guidelines.
Assist in the provision of training where required and ensure that Reablement Assistants are inducted effectively, in accordance with the agreed programme.
Liaise with the Call Centre and to assist in daily and weekly programming changes for Reablement Assistants as required.
Undertake first visits to service users undertaking risk assessments and establishing service users’ needs in accordance with Care Dorset policies and procedures: Care Coordinators will carry out assessments in conjunction with individuals, their families, carers and other professionals.
About you:
Previous experience within a senior care role is desirable. Training and developmental support will be provided, provided the candidate has a level 2 minimum.
Full driving license and access to your own vehicle
Business car insurance required for this role
Diploma in Health & Social Care Level 2 or equivalent
Good knowledge of Care Panning processes and Essential Standards of Quality and Safety
Good knowledge of risk assessment process
Previous supervisory/management experience in similar setting
Organisational, supervision and time management skills
Ability to pass on knowledge in an enabling way
Ability to fulfil the travel requirements for the post
Ability to support and promote independence
