About the Role:

• Provide high-quality support and care to adults with learning disabilities, promoting their independence and well-being.
• Collaborate with individuals, their families, and other professionals to develop and implement person-centered care plans.
• Conduct thorough assessments to identify needs and support individuals in achieving their goals and aspirations.
• Advocate for individuals' rights and access to appropriate services and support.
• Contribute to a positive and supportive team environment focused on client-centered care.

Requirements:

• Registered with Social Care England.
• Minimum 3 years' post-qualification experience as a Social Worker.
• Experience working with adults with learning disabilities is highly desirable.
• Excellent communication, interpersonal, and case management skills.
• Commitment to person-centered care and ethical practice.
