Social Worker - Adults Locality Team

This role offers an exciting opportunity to contribute to a team dedicated to supporting adults within a locality team.

Position description / Job description

  • Conduct comprehensive assessments of adults needs and develop person-centered care plans.
  • Provide direct social work services, including counselling, case management, and advocacy.
  • Collaborate with other professionals, such as health care providers and family members, to coordinate care.
  • Maintain accurate and up-to-date case records and documentation.
  • Participate in team meetings and contribute to the development of policies and procedures.
  • Engage in community outreach and education initiatives.
  • Advocate for the rights and well-being of adults in need.
  • Monitor and evaluate the effectiveness of interventions and make necessary adjustments to care plans.

Requirements

We're looking for you, if:

  • You hold a relevant social work qualification (e.g., MSW, LCSW).
  • You possess a minimum of 2 years of experience working with adults in a social work capacity.
  • You have a strong understanding of social work theories and practices.
  • You are proficient in conducting needs assessments and developing care plans.
  • You possess excellent communication, interpersonal, and problem-solving skills.
  • You are a highly organized and detail-oriented individual.
  • You are able to work independently and as part of a team.
  • You possess a valid driver's license and access to a reliable vehicle (if applicable).
  • Experience working within a locality team is advantageous.
