Social Worker - Adults Locality Team
This role offers an exciting opportunity to contribute to a team dedicated to supporting adults within a locality team.

- Conduct comprehensive assessments of adults needs and develop person-centered care plans.
- Provide direct social work services, including counselling, case management, and advocacy.
- Collaborate with other professionals, such as health care providers and family members, to coordinate care.
- Maintain accurate and up-to-date case records and documentation.
- Participate in team meetings and contribute to the development of policies and procedures.
- Engage in community outreach and education initiatives.
- Advocate for the rights and well-being of adults in need.
- Monitor and evaluate the effectiveness of interventions and make necessary adjustments to care plans.
Requirements
We're looking for you, if:
- You hold a relevant social work qualification (e.g., MSW, LCSW).
- You possess a minimum of 2 years of experience working with adults in a social work capacity.
- You have a strong understanding of social work theories and practices.
- You are proficient in conducting needs assessments and developing care plans.
- You possess excellent communication, interpersonal, and problem-solving skills.
- You are a highly organized and detail-oriented individual.
- You are able to work independently and as part of a team.
- You possess a valid driver's license and access to a reliable vehicle (if applicable).
- Experience working within a locality team is advantageous.
