Are you a skilled and compassionate Social Worker with experience in supporting adults with disabilities? Lincolnshire County Council is looking for a committed Adult Social Worker to join their Disabilities Team, making a real difference in the lives of adults with complex needs.

🔹 Role: Adult Social Worker - Disabilities Team
🔹 Location: Lincolnshire
🔹 Rate: £38 per hour
🔹 Contract: Full-time | Temporary (initial 3-6 months, with potential to extend)

🔍 About the Role:

You'll be working with adults with learning disabilities and/or physical disabilities to assess needs, promote independence, and ensure access to the right support and services. The role requires a strengths-based, person-centred approach in line with the Care A#####014 and relevant legislation.

✅ Key Responsibilities:

  Complete Care Act assessments, support plans, and reviews

  Coordinate care packages and liaise with providers and health professionals

  Carry out Mental Capacity Assessments and manage#####rong data-s#####="1313" data-end="1338">safeguarding concerns


Promote independence and community-based solutions
Maintain accurate, timely records and reports in line with statutory duties
