Description:

Carmarthenshire Council is seeking Adult Social Worke to provide high-quality support. This position involves supporting safe and timely discharges from hospital, working closely with health professionals, patients, and their families to ensure appropriate care plans are in place.

Requirements:

• Registered with Social Care Wales
• Up-to-date DBS check
• Over 2 year of relevant experience in social work within the UK
• A degree in social work or a related field
