as being homebased
.
This role is a full time role working 37 hours per week on a 12 month Fixed Term contract.
The successful candidate will receive a salary of £41,591 per annum.

KEY RESPONSIBILITIES
Develop opportunities and market Trust services in relation to Health and Social Care with a view to increasing the number and range of care and health referrals to the Trust.
Complete assessments on potential clients referred to the Trust for care and health placements.
Cost up each referral and ensure the placement is planned and well managed
Attend and represent the Trust at Tribunals, Parole Board hearings, Court and MAPPA meetings.
Ensure assessments and placements consider at all times risk management and identify the particular health and care needs of the client and identify how these will be met.
