Please note that the salary for this role is made up of a basic salary of £24,395 to £31,131 per annum plus a 6% guaranteed allowance as the Discharge to Assess Teams work over a 7-day week. This entails working one weekend in six (Saturday and Sunday, between 8am and 6.30pm, depending on your working hours), and one bank holiday a year to meet the team requirements.
As a Community Support Worker, your role will include undertaking Assessments and Reviews in line with our Statutory duties under the Care Act to identify eligible needs and to then generate a person-centred and outcome focused support plan. An essential attribute for the role is to be 'curious' as thinking 'outside the box' is a must to achieve the best outcomes and to fully engage and utilise all that our community has to offer. Other roles include supporting with the team's duty function, supporting as appropriate with safeguarding intervention and multi-agency working.
Partnership working with both internal and external partners is essential to achieving the best outcomes for Adults and Carers. As a Community Support Worker, you will need to work proactively to develop sustainable relationships with internal and external colleagues.
The work covers all ages (18+) and specialisms and is varied and interesting. An induction and appropriate training will be provided. You will also be joining a supportive team that encourages development and growth. It is also really important that you demonstrate a commitment to learning and development to support yourself and the team to deliver the best service possible.
About You
You'll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis. You will have the drive and passion to want to make a difference, and a willingness to learn. Ideally you will have some experience of activity-based work with adults and some knowledge of legislative and policy guidance applicable within social care settings.
