Reading Borough Council

I am a highly motivated leader in Adult Social Care with a proven track record of working in collaboration with Health and other partners at strategic and operational levels to deliver efficiencies and improved outcomes for residents. I bring a wealth of experience and in-depth knowledge about the challenges and opportunities currently facing local government, partners and communities. I have led substantial strategic and operational improvements in a range of service areas during the last 6 years. I have worked collaboratively with Health partners to develop culture, processes and pathways to improve hospital discharges and prevention of admission. I have provided strategic and operational leadership to implement a strengths based approach in the access team, adopting a conversations approach and utilising reablement to maximise independence. I have worked with CCG, PCN Clinical Directors and Community Trust partners towards redesigning services around neighbourhoods to improve health inequalities. Having worked in Reading Borough Council for 16 years and in my current role for four years, I am ready to bring my skills, knowledge and abilities to a new role and to continue in my professional development. KEY SKILLS * Leadership skills at strategic and operational levels, ability to identify opportunities to implement operational changes, gaining engagement from others to redesign and implement changes to achieve strategic goals. I led the introduction of Social Care Occupational Therapy into the Reablement Team to achieve the strategic goals of enabling people to remain independent for longer and reduce the need for Social Care and health provision, gaining funding agreement from an initially resistant CCG. This has resulted in improved outcome measures and significant savings. I led the creation of a hospital discharge team, collaborating with system partners to develop a pathway and home first culture to improve performance in relation to national and corporate targets. * High level of organisation, agile and adaptable, able to manage competing and changing priorities in a calm and measured way. Personal resilience and ability to cope with highly challenging situations. During a three-month period, I successfully led the Local Authority engagement in a CQC review, and restructures of two services, in addition to my ongoing responsibilities. * Able to lead change, engaging others and gaining trust by clearly communicating and sharing the vision, aims and objectives. I foster a positive and open culture by listening to concerns and anxieties and enabling co-production. I led ongoing improvement in performance in delayed transfers of Care, through working with system leaders, contributing to a weekly strategic meeting, sharing performance data and targets with the operational team and engaging them in identifying solutions. * Extensive experience of successful partnership working, active participation in many system activities with these partners including a monthly Integration Board and Urgent and Emergency Care Board. I have worked with a CCG partner to gain the support of public health, the community health trust, clinical directors and voluntary sector to drive forward a population health management and a multi-disciplinary approach to addressing health inequalities and improve efficiency. I have worked with colleagues in health to implement changes to processes at a system level, most recently implementing the COVID 19 discharge requirements. * Excellent relationship management and networking skills which I have demonstrated in my work with system partners, establishing trust with partner agencies especially health colleagues to effect operational change in line with strategic goals and bring about efficiencies and improved outcomes for residents. I led the local authority contribution to a CQC review of the Health and Social Care system in 2018, working intensively with partners to jointly compile information and engage actively in the review. The review identified that `Reading residents receive good quality, person-centred health and Social Care when they need it, from compassionate, dedicated staff.' * Excellent analytical skills and I can evaluate options, innovate and identify solutions. Having analysed the outcomes, costs and best practice in relation to the bed- based discharge to assess service, I identified significant benefits in customer service and cost effectiveness which could be gained through a re-provision of the service. I led on the restructure of the service, reducing it from 14 to 4 beds to reflect the demand that I had identified and to reflect the Home First agenda. The service moved to an environment which provided much better opportunities to achieve reablement goals and delivered savings of circa £ 1million. * Extensive up to date professional knowledge of Adult Social Care and Health service provisions, including an applied knowledge of Social Care legislation, experience of advising others and chairing a decision-making group. I have experience of high risk, multi-faceted situations and the ability to advise others how to identify resources and collaborate with other organisations taking a strengths-based approach, following safeguarding procedures and making safeguarding personal. * In-depth knowledge of community and acute health services gained over the last 4 years of strategic partnership working. I understand the interdependence of these service provisions at all levels from the promotion of health and wellbeing and healthy communities, community health and Social Care provision, acute health Care, and the role of other fields such as public health and the voluntary sector.
