As well as working independently on some specific responsibilities, the role also requires me to work closely with other members of the team to complete other tasks and provides opportunity to contribute to the team achieving collective performance standards and targets. I have also been responsible for the training of new members of staff, ensuring they are supported to complete their tasks successfully and are shown best practice throughout, as well as providing a warm welcome to the Santander team. I am also an experienced Health Co-ordinator, having coordinated and delivered the National Diabetes Prevention Programme across the North East of England for two major firms who had been awarded this contract by the NHS. The Health Co-ordinator role was multi-faceted and involved a combination of line management, logistics co-ordination, relationship management and performance management. I line managed a team of Health Educators delivering the programme and was responsible for the overall availability of the programme, ensuring that it was delivered in the most appropriate and beneficial locations and venues for participants with a focus on ensuring patients had every opportunity to successfully complete the programme. Prior to taking on the role of Co-ordinator, I gained significant experience as an Educator, delivering the National Diabetes Prevention Programme to hundreds of patients considered at risk of developing Type 2 diabetes throughout the North East of England. This role involved working with patients within group and individual settings to enable them to develop their understanding of how to live healthy lifestyles, make informed modifications to their diet and exercise levels and in doing so, raise their confidence and empower them to take positive control of their lifestyle. I was responsible for sourcing and securing local venues, records management and marketing of the programmes via both large and small-scale presentations. Educated to degree level in psychology, I enjoy applying my knowledge of psychological processes and approaches, as well as my interpersonal skills, to support and motivate others, encourage improvements/change, understand concerns and develop ways to address them. I have significant experience of planning and scheduling, working with both colleagues and stakeholders to meet delivery targets and ensure that all records are kept up to date and proper communications are well maintained. I have considerable experience of working with the public in a number of settings including health care and retail settings and in every role, I am proud to have received outstanding feedback from customers/patients. Key Achievements * Having achieved extremely positive feedback from groups in my initial role at Living Well, my hours/duties were significantly extended to work with groups across Durham city, Newton Aycliffe, Peterlee, Barnard Castle, Stanhope, Framwellgate Moor, Bishop Auckland and Chester -le-Street. Throughout my employment with Living Well, I received excellent feedback from groups I worked with, and achieved significant success rates in terms of patients' goals being met. * I have received numerous letters from individual patients expressing gratitude and commending my approach to supporting groups and individuals. * As a member of the Living Well team, I was asked to deliver a key presentation to a large number of primary care colleagues from across the local health community about the health educator role, the processes involved and the benefits. The session received very positive feedback and referrals increased as a result.
