DURING MY TIME WITH THIS FAMILY I WORKED WITH VARIOUS HEALTH PROFESSIONALS AND LIASED REGULARLY WITH THE SOCIAL WORK TEAM. DURING THIS TIME I ALSO MADE SURE I DOCUMENTATED AND WROTE VALID REPORTS INLINE WITH THE GIRFEC TOOL.
